Health check microflow does not result in an email being sent

We have implemented a health check microflow in the project settings, and deployed to Test. We can see from the logs that the flow works as designed and generates a non-empty output string when something is wrong. However, nobody is receiving an email alert. Even though the box to receive alerts is ticked in Sprintr for the Test environment. What could be the reason why we're not receiving an email?

In my experience, the health check microflow needs to fail four consecutive times before the status of your environment changes. When the status changes, you should get an email.
